Situation, Background, Appearance, Review (SBAR) and Notify. Provides a comprehensive and user-friendly nursing evaluation that helps guide and document critical thinking in an efficient way. SBAR prepares nurses to provide more detailed information about resident conditions and in turn aids practitioners to be better informed when determining next steps.

DECISION SUPPORT TOOLS: CHANGE IN CONDITION FILE CARDS AND CARE PATHS

These tools were adapted as guidance from the AMDA Clinical Practice Guideline on Acute Changes in Condition in the Long-Term Care Setting. They provide criteria for nurses to determine what signs/symptoms need to be reported to the medical practitioner immediately and what requires non-immediate notification. These tools can be available at the nurses’ station as a quick reference guide.
