In honor of Alzheimer’s disease and Dementia Care Staff Education Week February 14th to the 21st, the NCCDP is accepting nominations for Educator of the Year

The National Council of Certified Dementia Practitioners is seeking nominations for the Annual Educator of the Year Award. The Award will be granted to an educator who has made a significant contribution to the quality of life of the resident’s / client / patient, longevity of your staff and decrease in staff turnover due to the educational programs developed and implemented by your educator. This award is also open to educators working in learning institutions that provide education to students enrolled in the learning institution and entering the health care industry.

General Instructions and Information:

The nominee must be a Certified Dementia Practitioner and Certified Alzheimer’s Disease and Dementia Care Trainer CADDCT that has provided education programs for a minimum of three years and in good standing with the NCCDP.

  • All information is confidential and will not be returned.
  • A recent photo must accompany the completed application.
  • All applicants will be informed of the decision by the NCCDP Awards Committee by March 30th.

Please complete and mail the nomination form attached to this mailing. The nomination letter and form are also located on the www.nccdp.org web site in the Alzheimer’s disease and Dementia Care Staff Education Week Tool Kit. See blinking icon.

To nominate an individual for this award, please complete (typed only) the application form describing how the individual meets the award criteria.

All applications must be received by March 15 via email only.
