Improper Principal Hospice Diagnosis Code No Longer Accepted by the LTC Online Portal When Submitting Form 3071Information posted July 31, 2015To ensure accurate federal reporting of Hospice forms and claims, beginning August 28, 2015, the Long Term Care (LTC) Online Portal will no longer allow submissions with an improper Principal Hospice Diagnosis Code in field 13 of the Texas Medicaid Hospice Program Individual Election/Cancellation/Update Form (Form 3071) as indicated in the Department of Aging and Disability Services (DADS) Information Letter 14-51. If an improper Principal Hospice Diagnosis Code is entered in field 13 of the LTC Online Portal, providers will receive an error message stating “Diagnosis Code in Field 13 is an improper Principal Hospice Diagnosis Code,” and the form will not be accepted.
Note that 3071 election and update forms submitted prior to August 28, 2015, containing one of the improper The table below lists the ICD-9 and ICD-10 principal diagnosis codes that the LTC Online Portal will not accept
