INTERACT Implementation Guide
This guide includes evidence-based and expert-recommended tools, strategies to implement them, and related educational resources. The INTERACT tools are meant to be integrated into every day care and be incorporated into your facility’s quality improvement program and processes.
Quality Improvement Tools
The Quality Improvement Tools are fundamental to the INTERACT program. The Hospitalization Rate Tracking Tools and the Quality Improvement Tool s are designed to assist in two of the basic components of a quality improvement program: 1. Tracking, trending, and benchmarking clear and consistently defined measures; and 2. Conducting root cause analyses that identify areas for improvement. Summarizing the findings from Quality Improvement Tools over time should help focus educational and care improvement activities.

Quality Improvement Reviews – Root Cause Analyses:
The INTERACT Quality Improvement Tool is designed to help your team analyze hospital transfers (including ER visits, observation stay and admissions) and identify opportunities to reduce transfers that might be preventable. Complete this tool for each or a representative sample of hospital transfers in order to conduct a root cause analysis and identify common reasons for transfers. Examining trends in these data with the INTERACT Quality Improvement Summary Tool can help you focus educational and care process improvement activities.
